How to avoid it and fix it?

Solution:

This message appears when the branch piping is exactly collinear with the header pipe (on the same axis as header pipe)

As one can clearly see in the above image, the branch piping (highlighted red) is on the same axis (ex. X-axis) as the header pipe (ex. Seg A). Therefore, Tee branch piping (Seg B) is collinear with the Tee header pipe (Seg A).

Avoidance:

To avoid having this message displayed, model the branch at any axis than that of the header pipe.
