Own changes and extensions can easily be made via Customize Ribbon, with any dgn open.
For example, as described under c) Tabs Groups and Tools.

These changes are not saved in the workflow file, but in your settings.
This can be seen well in the following screenshot. At Customize volume above in the title the file Personal.dgnlib is listed and not the PS_Compact dgnlib.
All personal changes are then shown in bold, no matter if tools are added or deleted.

The whole thing then has two advantages:

Update workflow

This means that a more current PS_compact file can be loaded later without losing your changes.

Save changes

You can also save your changes separately or copy them to another computer.
This is then the file Personal.dgnlib.
It has the same file location as Personal.KeyboardShortcuts.xml and to find this folder is described in the wiki page 5 Setting up PS_compact.
