Why and how to avoid it?

Answer: 

When using the tank module, the applicable codes usually require that bottom plates be added into the model before running an analysis. 

Confirm bottom plates are defined by looking at the bottom of the tank in the 3d view. 

Avoidance:

Simply inserting a bottom plate before running an analysis.
