Problem:
Why does Max Ratio = 0.00 and Notes = Fail for flange in AutoPIPE Flange analysis module?

Why and how to Avoid it?
Solution:
There could be a number of reasons for this to occur. This may be an iterative operation requiring more than one solution listed below:
Solution #1:
Confirm that on the Non-Code combination dialog that all combinations have the Print flag enabled / checked ON.
Open Flange Analysis module and press the Analyze button again. If all the other flange information is completed than the Max Ratio should be grater than 0.00
Solution #2:
Using AutoPIPE v.10.01.00.08 where Flanges max ratio was blank and physically connected to a valve or have any type of extra data assigned may be affected by a known error; Defect 263628 see here. Follow suggested Avoidance steps on linked WIKI page.
