Problem:

Issues created in Visualize Results dialog do not show up in the Issues column. 

Solution:

Only forms created using the Clash form type will show under the Issues column. 

If a Clash form does not exist create a new one in the https://connect-formmanager.bentley.com/designer create a new form using Clash Type. 

Once created use the Clash form when creating new issues in Visualize Results.
