Survey FWD is not loading information into the InRoads Fieldbook

Problem: 

When opening an FWD file, they are receiving the alert that "One or more Target Heights Missing from File" and the survey fieldbook doesn't contain all the information that was there when they previously saved it.  The FWD is loaded just fine on other machines with no alerts and all the information is in the fieldbook.

Solution:

When opening an FWD file, the program is using a Text Import Wizard (Survey.TIW) to read the format of the FWD and load the information.  Somehow the TIW is corrupt so when the file is opened, the program cannot read the survey information correctly from the FWD.  Attached is the default Survey.TIW file.

Extract the Survey.tiw file from this compressed zip file and replace the existing Survey.TIW located at C:\Program Files (x86)\Bentley\InRoads Group V8.11\Data.  Now when you open the FWD it should use a good wizard to format the information and open the fieldbook correctly.
