After logging in and selecting the desired project, choose 'Type Note.' 

Write out the appropriate text then tap 'Save,' located in the top righthand corner. 

Next, the application with prompt you to attach a snapshot or video either new or existing. 

You can take a photo with the camera right then and there with new snapshot and video, or select an existing from the gallery of the phone. 

After uploading all necessary pictures hit 'I'm Done' and the Note will be uploaded.
