After logging in and selecting the desired project, choose 'Type Note.' 

Write out the appropriate text then tap 'Save,' located in the top righthand corner. 

Next, the application with prompt you to attach a snapshot or video. 

You can go either route of capturing the photo with the camera with 'New Snapshot/Video' or choose 'Use Existing' to select from the phone's gallery.

 After uploading all necessary pictures hit 'I'm Done' and the note will be uploaded.
