You may have set the Use option to False in your Survey Project Settings.
Go to your Survey Project Settings Properties and look at your Data File Parsing>Data Import Items and see if you set it to False. Change it to True and Accept.
In the video it shows an option to import a civil 3d FBK in the menu list. I don't have that in my list of import file types. Maybe there is something I'm missing? A missing TIW file?
This does correct the issue with the slope distances and heights however the NEZ coordinate records are "translated" to US Survey feet based on the incorrect units in header of the FBK. This should be fixed so the correct units can be used in the header.
I have gone ahead and filed a defect for this. The defect number is 292113 and will be placed in the development queue for the civil quarterly software updates

I'm using Open Roads Power Geopak 08.11.09.845. The problem still persists. Would be good if the issue was corrected. Our firm shares the FBK file format across all survey projects and all clients. Fixing this would make the FBK files universal between Civil 3D and Open Roads workflows. Here's to hoping!
Yes I get the same results.
I use the FDOT seed files. My design file settings are above. I don't see anything there. I really need this resolved so please keep brainstorming!

I was able to reproduce this issue, I will log a defect for this.
Do I attach dgn files using the secure file attachment process? I understand how to attach the media files but not dgn files.
What are your Design File Working Units?

Change the header of the FBK to read :
UNITS FOOT DMS
Instead of
UNITS USFOOT DMS

Does it provide the same results when you do not use the FDOT workspace? You may want to attach your files

How do I get a copy of the Civil 3D FBK tiw file you have highlighted above? This format is not present in my settings dialog.
